Has any landlord in Florida filled this out? I am in the process of asking the Judge to award a final default judgement. So apparently I need to fill in two forms a. a motion for default final judgement and an Affidavit for damages. I have a couple of questions regarding the affidavit:

a. Do I only enter the amount as listed in the original complaint at the time the eviction process was initiated? 
b. Or do I enter the total damages because the tenant continued to live rent free during the eviction process?

Also do I need to get it notarized only by a Florida notary? I live out of state in NY. Can I get a NY notary to sign off on my affidavit?

I would think that they would want the total of all monies owed at the final monetary hearing.  Not an estimate filed before the tenant left.  And I don't know if there is a difference between being notarized in one state or another.  Isn't being notarized the same all over?  But you do need to get FL specific answers on this question.  Find a landlord who has done this before and ask him/her.  Look through the last couple of weeks of court cases and find another owner who filed this.
