A relative needs to give notice to long term renters, he can't remember if a lead disclosure was given in 1995 when property was rented. Property management company said he could be sued or fined by tenant because no lead disclosure was given to the renter in 1995. Is this true?

He should be ok if he hasn't done any new lease with them or renewed their lease (leaving them month to month) since they first moved in.  It wasn't required for single family housing until 1996. 
However, he should be sure to give out this form with any new tenants.
