I suggest you borrow a book from the library entitled "Every LL's Legal Guide" from NOLO Press.  It has all the blank forms on a disc that you can download and edit.  For personal use in your own rental properties, there is no problem with using their application form.  If you were using it for a property mamangement business, you may need permission to use it.

My library is currently out of stock on that book.  Any other suggestions on where to get an application form?  Does Officemax and the like sell them?

I found an online rental application form that works well for me:

Just in case anyone need to fill out a rental application form, I found a blank form in this link http://pdf.ac/6LKce. This site PDFfiller also has several related forms that you might find useful.

Hey everyone,

I was curious about this subject as well. Figured I'd post in this thread instead of starting a new one.

I noticed that there are many different types of rental application forms all over the net. Here are couple other examples:

I was wondering if there were any major differences, or key things I should be looking out for when making mine? I'm from South Carolina, do these documents have any state-by-state requirements? Or are they mostly the same across the board and it's not a big deal?

Info you will need on an application is the full name of everyone to live in the unit (including maiden and other names used), DOBs, SSNs (of adults), addresses (current and several previous if they move a lot), landlord names and phones, dates rented and rent amounts, employment (current and former), phone number for work, amount of pay, dates worked, pets?, smokers?, other income received, names of banks, loans they pay, if they are current military members, if they have filed bankruptcy (& when), if they've been evicted, been convicted of felonies or repetitive misdemeanors, if they've ever lost a deposit, next of kin, ID type and number, and an authorization to check their references and credit above the signature lines.
